| COMMUNITY CARE Definition: (British English) Medical and other care for people who need help over a long period, which allows them to live at home rather than in a hospital. ______________________________________________________________________ * All eleven elements should be in. 1. Purposeful - to provide care and help ! 2. Necessity and deliberate to keep patients at home. 3. Home environment is considered better than the hospital's towards helping patient to recover more quickly, in the opinion of the authorizing (medical, etc.) professionals. (Connect to #10 & #11.) 4. There are loved ones living with or close to the person/people left to recover at home. (To emphasize.) 5. Care and help that is provided is of the appropriate kind and of the best grade. 6. Providers of care, that is, medical staff, relatives and the other professionals, are qualified, diligent and effective. That is, patients are not taken advantage of because they are not in a (the) hospital. 8. Cost of care is not prohibitive or higher than if person was in a hospital, as an in-patient. 9. Home environment is safe, adequately protected and secured, in addition to being very clean and tidy (to emphasize). 10. Care lasts for appropriate duration - as long as needed (connect to #11). 11.
